PLEASE ANSWER ALL QUESTIONS UNLESS YOU'RE PRE-REGISTERING AS A NON-PLAYING CRUISE PASSENGER ONLY AND NOT SHARING A CABIN WITH A TEAM USA PLAYER.  SUCH NON-PLAYERS NEED TO ANSWER QUESTIONS 1-5 AND 17-20 ONLY.
